As we age, most of us will find ourselves transitioning from home to the hospital and then from the hospital to a skilled nursing facility. These transitions can be stressful, but that stress can be alleviated if we prepare for the transitions. Dr. Gordon Glasgow, the medical director of Palm Terrace Healthcare and Rehabilitation, gives three excellent tips to help patients make a smooth transition from home to the hospital and then to the skilled nursing facility.
- Make sure your vaccinations are up to date. That’s one less thing to worry about!
- Tour the facility in advance if you’re going in for a surgical procedure. This will help you feel comfortable and know where to go.
- Talk with your primary care doctor so you know what to expect. It’s comforting and empowering to understand what sort of care you will be receiving.
Following these three tips will help you receive the best possible care—from your caregivers and yourself.
